The building, the Royal Hospital Kilmainham, was completed in 1684 as a home for retired soldiers and served this purpose for over 250 years. It is one of the oldest classical buildings in Ireland. Following extensive restoration in the 1980s, the museum was established and officially opened to the public in May 1991.

The Royal Hospital was originally built on the site of a 6th-century monastery and later a 12th-century priory of the Knights Hospitaller. A verified historical feature is the "Bully's Acre," Dublin's oldest cemetery, located on the museum grounds, which contains the grave of Dan Donnelly, a famous Irish bare-knuckle boxer.
