The Holy Trinity Cathedral is the oldest church in Ruse, located in the city center adjacent to the Opera House. It is a unique example of underground religious architecture from the Ottoman period.

The church is a "semi-buried" structure, with its floor level situated 4.5 meters below the modern street level. Externally, it appears as a low, modest stone building, a design mandated by Ottoman laws requiring churches to be no taller than a man on horseback. The interior features a three-nave basilica layout with a richly carved wooden iconostasis and murals added in the early 20th century. A separate hexagonal bell tower, built after 1878, stands prominently in the courtyard.
