The cathedral is a brick Gothic hall church featuring two symmetrical 73-meter neo-Gothic spires added in the late 19th century. The exterior is composed of exposed red brick with stone decorative elements and buttresses. The interior contains late-Baroque altars, a 15th-century Gothic font, and a specialized triptych in the side chapel.

The current brick structure dates primarily to the 15th century, though it stands on the site of previous Romanesque churches dating back to the 11th century. It was commissioned by the Piast Dukes of Opole. Significant reconstruction occurred following fires in 1615 and 1739, and the towers were completed in 1899.

According to local tradition, the church was founded in 1002 by Bolesław I the Brave. A specific stone near the entrance is said to bear the footprint of Saint Adalbert, who allegedly preached on this site during his missionary journey to Prussia in 997 AD.
